Facility Director of Nursing Service
Department: Nursing Supervisor: Nursing Home Administrator Salaried
Job Summary:
The Director of Nursing Services is responsible for directing the activities of licensed and non-licensed personnel who provide health care & nursing services to Residents.
Description:
Under the supervision of the Nursing Home Administrator, the Director of Nursing directs the operations of the Nursing
Service Department to ensure that the highest degree of quality Resident care is maintained at all times in accordance with
State and Federal Regulations, Facility policies & procedures, and Resident directives. This position helps to ensure that the total needs of Residents are met through the development of the Minimum Data Assessment (MDS) and Resident Care Plan coordination.

Qualifications:
A. Graduate of an accredited school for professional nurses and licensed as a Registered Nurse in good standing in the state where work will be performed.
B. Associate degree (A.A.) or equivalent from two-year college or technical school, or six (6) months to one (1) year related experience and/or training, or equivalent combination of education & experience.
C. Satisfactory OIG exclusions and background check.
D. Satisfactory personal & professional references.
E. Possess & maintain current CPR certification or acquire CPR certification within 90 days of date of hire.
Work Schedule:
Work scheduled shift and as needed at other times, Sunday through Saturday. Works beyond normal working hours & on weekends and holidays when necessary. On-Call 24 hours per day, 7 days per week. A minimum of 40 hours is required.
Comp time is not offered, meaning that the Director of Nursing must work as many hours as necessary to ensure all required work is accomplished and working more than forty (40) hours in a workweek does not result in the Director of
Nursing becoming eligible for any additional time off as compensatory time or additional vacation time. Participates in the
On Call schedule rotation, MOD rotation, and smoke schedule. Picks up shifts on the floor when necessary to avoid agency use in the Facility. If there is any time missed from a scheduled shift, PTO will be applied to the time missed in accordance with the Company’s PTO policy.
Department Managers & governmental agency representatives;
- Maintains a liaison with the community referral sources, Residents, families, support departments, etc., to adequately plan for Residents’ needs;
- Involved in community/civic health matters/projects as appropriate;
- Works beyond normal working hours on evenings, weekends, & holidays when necessary for completion of job duties and/or as scheduled by the Administrator;
- Is subject to call-back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.);
- Conducts, attends & participates in in-service programs & continuing education programs as outlined by state regulations & by Facility policy every payday & as otherwise scheduled by the Administrator;
- Is subject to injury, falls, burns from equipment, odors, etc., including TB, AIDS, & Hepatitis B viruses; as well as, reactions from dust, disinfectants, & other air contaminants – protective equipment provided;
- May be subject to the handling of, & exposure to, hazardous chemicals – protective equipment provided.

1. Nursing Administration a. Maintain the Nursing Services Program in “survey ready” status at all times; b. Direct operations & maintain compliance with State & Federal regulations affecting the Facility’s nursing functions; c. Coordinate, integrate & evaluate functions and activities of the nursing program; d. Coordinate implementation of methods & procedures to ensure achievement of objectives; e. Supervise 1-5 subordinate supervisors (e.g. ADON, Charge Nurse) who supervise nursing staff to include RNs,
LVNs, QMAs, CNAs, Medical Records; MDS Coordinator; Restorative Aides & others; f. Advise & assist Administrator, Medical Director, & medical staff in planning and developing administrative & operational policies/procedures, Facility use & planning, and strategic planning; g. Recommend establishment or revision of policies/procedures/methods & develops organizational structure and standards of performance; h. Establish written nursing procedures that ensure consistent delivery of care & maintain/promote Resident rights, under the direction of the Administrator.
2. Direct Care & Care Supervision
a. Participate in admission, transfer, & discharge determinations, including examination of the Resident clinical condition to determine appropriate care needs; b. Oversee clinical operations (e.g. monitoring Resident conditions, psychopharmacological drugs, restraint use,
Foley catheter protocol, pressure ulcer monitoring); c. Carry out ongoing evaluation of nursing care & the following of Facility policy/procedure. Make regular periodic rounds of Resident care units to ensure; i. Quality of care being provided; ii. Resident Rights being followed; iii. Confidentiality being maintained; iv. Cleanliness; v. Timeliness of staff response to call lights & Resident/family requests; vi. Appropriate positioning/safety of Residents; vii. Appropriateness of infection control technique usage; viii. Staff involvement & appropriateness; ix. Proper usage of equipment; x. Safety/Security of Residents & staff; d. Ensure that Residents are free from abuse, mistreatment & neglect; e. Direct the work of Charge Nurse(s) on each unit in the absence of an ADON; f. Direct the work of Certified Nurse Aids(CNAs) in the absence of an ADON and/or Charge Nurse; g. Confer with staff and/or Resident’s physician, as needed, to clarify medical orders & direct care.
3. Contribute to Facility efforts to maintain &/or improve quality of care through the following: a. Participate in the investigation of Compliance Hotline calls; b. Serve on designated committees; c. Hold nursing meetings; d. Attend Department Head & other supervisory meetings as called by the Administrator; e. Attend and actively participate in Facility meetings, such as staff meetings, in-services, & designated committee meetings; f. Promote quality assurance; g. Immediately investigate & report to the Administrator & appropriate governmental agencies any suspicions of Abuse, Neglect or Misappropriation of Resident property.
4. Miscellaneous a. Maintain conformity to Federal and State Regulations for Long Term Care Facilities; b. Review and adhere to all Facility Policy and Procedures; c. Maintain conformity to Facility Standards as required; d. Promote public relations; e. Perform other duties as assigned by the Administrator; f. Comply with uniform/attire (which includes name tag, appropriate shoes) and other requirements in accordance with policies and procedures of the Facility.
Access to Resident PHI
This position has access to Resident PHI & other information as needed to adequately assess Residents & provide/direct care of the Resident on an ongoing basis as part of the interdisciplinary care team. This position is responsible for the confidentiality of all information in accordance with Federal & State requirements and Facility policies.
