We are currently seeking a full-time Job Coach to work in our Hauppauge facility. Individual will be responsible for all job coaching service delivery to individuals with disabilities in all employment models (individual, enclaves, mobile, affirmative business) as per State, Federal and Agency Policy and Procedures.

• Responsible for all daily logs, travel, daily on-site reports, progress notes, unit tracking, quarterly reports, individual annual and semi-annual plans.
• Attend all required meetings and trainings.
• Works closely with consumer, families, referring agencies, service coordinators and other community employment and vocational staff.
• Must operate own vehicle between job sites and agency sites. May include driving agency van to transport persons served to and from interviews or job sites.
• 40 hours/week (approx 8a-4:30p Monday to Friday; however, days may be flexible depending on persons served - may include early hours, weekend hours and evening hours (only if required).

Requirements:

High School Diploma or GED and 1-3 months related experience.

Clean and valid NYS Drivers License

Current First Aid Certification or ability to obtain within 3 months.

Salary Range

$17.00 - $18.70/hour depending on experience
