JOB SUMMARY:
Provide leadership and professional social services to patients and families receiving care and/or services at Grand Itasca Clinic and Hospital. Organize health care services to promote optimal outcomes for individuals utilizing resources, payor sources and clinical data. Assist in meeting the psychosocial needs of patients and to utilize medical care in order to achieve their optimal level of health. The social worker will serve as a patient advocate. They will assist in the identification and utilization of internal and external referral sources to augment or meet the patient needs. Services provided will be in accordance with current federal, state and local standards. Social Services will promote utilization of health care resources and provide a leadership role at Grand Itasca Clinic and Hospital.

JOB DUTIES:

• Provide direct social work service and counseling to patients and their families and/or groups of patients.
• Social and practical environmental problems including identification of those needing financial assistance, and either assisting or referring to the patient liaison, seeking financial aid, securing a homemaker, arranging for home care, discharge planning including collaboration with community agencies, and referrals to other community agencies for specialized help.
• Determines social assessments of total patient needs.
• Determines the patient's social, psychological, financial, cultural and family situation.
• Obtains resources from community social, health and welfare agencies.
• Provides consultation to members of hospital staff, community agencies, and other persons or groups seeking guidance in efforts to solve the problems of patients.
• Participates as a member of the health care team for the purposes of care planning and delivery.
• Advocates for the patient.
• Monitors and reports any and all vulnerable adult or abuse/neglect concerns to the appropriate agency and assists in the investigative phase as necessary.
• Works in conjunction with Utilization review staff to ensure patients' needs and criteria are planned for and met.
• Assists with Advanced Directives as needed.
• Responsible for the complete orientation and maintenance of records of all staff, along with any mandatory education.

QUALIFICATIONS:

• Master's degree in Social Work, preferred.
• Current licensure in State of Minnesota through the MN Board of Social Work.
• Acute care experience preferred.
• Demonstrated effectiveness in leadership role with ability to elevate the support of subordinates and peers.
• Demonstrates respectful and effective communication skills with patients, families, physicians and staff.
• Knowledge of and active participation in the PI process.
• Demonstrated ability to effectively problem solve in difficult situations.
• Ability to facilitate admission and discharge planning with the Interdisciplinary Team, patient, and family.
• Demonstrated and consistent ability to follow the NASW Code of Ethics.
• Demonstrated ability to follow and adhere to Grand Itasca Clinic and Hospital policies, vision, mission and values.
• Maintains reliable and consistent attendance.
