Gulf Coast Center - Job Description:
- Job Title: Licensed Vocation Nurse (LVN).
- Location: Based in Texas City, TX. Works in Galveston and Brazoria County (company vehicle use when traveling between job sites when one is available. If one is not available, we provide mileage reimbursement for use of personal vehicle).
- FLSA Status: Non-Exempt.
- Working Status: Full Time with Benefits.
Job Description Summary:
- Reports to the Program Manager, YBHS with oversight from the CNO.
- Responsible for working with youth with a diagnosis of a mental illness with evidence of serious emotional, behavioral, or mental disturbance and/or serious functional impairment.
- Assist in the delivery of comprehensive behavioral health services to Center-established patients; demonstrates proficiency in all aspects of nursing care; participate in the coordination of the medication clinic, support prescribing providers, & facilitate patient management visits; responsible for the monitoring & scheduling of lab studies and injections where applicable; responsible for the accurate and safe administration of injectable medication where applicable; responsible for medication reconciliation; conducts biometric screening and provides patient & family education where appropriate; cross-trained to provide adult nursing staff coverage/back up and back up for completion of Drug Prior Authorization Requests for the youth population.
- Promotes clear communication amongst care team & treating clinicians by ensuring awareness regarding patient care plans; participates in team discussion and monitoring of medical needs for individuals with behavioral health needs; completes referrals as needed; documents all activities and communications according to the Center's clinical documentation guidelines.
- Treats all individuals with the utmost courtesy, decency, and respect.
- Provides the highest level of Customer Service when interacting with staff, consumers, local entities, and advocacy groups to provide a service that meets local needs.
- Strict adherence to the professional, ethical, and legal requirements outlined in GCC's Business Code of Conduct as well as the Corporate Compliance Plan.
- May be required to travel within Brazoria and Galveston Counties.
Required Education:
- Requires graduation from an accredited high school or equivalent.
- Requires graduation from an accredited school of nursing as approved by the Texas State Board of Nurse Examiners; licensed as an LVN in the state of Texas.
Required Licensure/Certifications:
- Must maintain current licensure through the Texas State Board of Nurse Examiners.
Required Experience:
- ACCEPTING RECENT GRADUATES!
Preferred Experience:
- 2-5 years of full-time paid experience in a clinical setting.
- Experience in general nursing care of patients, preferably extensive experience working with individuals with behavioral health disorders.
Working Conditions:
- Work will be performed in the office setting.
- May require varied work hours depending on the needs of clients.
- Out-of-region travel may be required to attend training.
- May require travel to various locations within Brazoria and Galveston Counties. Requires current Texas Driver's License, reliable transportation safe and current with all state requirements, and liability insurance.
- Standard office/clinic environment.
- Work hours are generally 8:00 am to 5:00 pm, Monday through Friday, but may require irregular work hours.
- At the time of reduced staffing, and under the direction of the CNO, may be assigned to any of the four GCC behavioral health outpatient adult clinics to assist with coverage needs.
- Office & fieldwork, to include community setting; patient home visits.
- May be asked to participate in after-hours on-call rotation.
