The Position

This position will serve as the nurse (RN) Care Coordinator for people in our CCBHC program, providing professional psychiatric nursing services and care coordination to individuals with complex conditions (physical and behavioral health needs) being seen in SCBS’s several Certified Community Behavioral Health Center programs. Responsible for the care and treatment of these individuals in accordance with accepted nursing and medical standards. Working closely with program staff to attend to the whole health needs (physical, mental and substance use conditions) of people in our programs. Interacting with participants using person centered, family centered principals in a therapeutic manner and providing direct services in the area of medication, care coordination, independent skills training and behavioral and physical health education. Working productively and effectively with all members of the treatment team in providing interventions appropriate to the recovery/care plan for the individuals served. Carrying out assigned treatment plan strategies and documents these activities as per program procedure. Closely collaborates with other SCBS staff members and community stakeholders regarding outreach and wellness activities that can best serve the needs of our patients. Travel to local and rural areas. Company vehicle provided. To perform this job successfully, an individual must be able to perform each essential duty in a satisfactory manner. This position will require a flexible work schedule to accommodate for the needs of participants in the program. Graduation from an accredited school of nursing and licensed to practice in the State of Nebraska. One year of experience in medical/surgical nursing with an additional year of experience in psychiatric nursing preferred. Valid Nebraska driver’s license required. Using the link provided

Pay: $16.00 - $18.00 per hour
