Job description
Conviva Care Solutions is seeking a hard-working and dedicated Registered Nurse to promote and restore patients' health by completing the nursing process and who is interested in being part of a team that focuses on excellent service proactively outreaching to high-risk patients.
Preferred Qualifications
- Licensed Registered Nurse with no disciplinary action
- Ability to work independently under general instructions with a team with good communication and interpersonal skills with the ability to quickly build rapport
- Ability to work with minimal supervision within the role and scope
- Prior clinical experience in ER, Hospital and/or ambulatory patient education
- Prior Call center or Triage experience
- Prior clinical experience in case management a plus
- Intermediate knowledge of Microsoft Word, Outlook, Excel and prior EMR exposure plus
- Bilingual is a plus
Education:
- Associates Degree or Bachelor of Science Degree in Nursing
- Nursing Diploma
Working hours: Monday to Friday 8AM-5PM, remote with 1-2 times monthly in center for meetings, needs availability of a private home office workspace
